Used to report important information about your company's status, the operation of your plan, and the goals and objectives of your plan.

» Beneficiary Designation Package (for plans without Qualified Joint and Survivor Annuity)
» Beneficiary Designation Package (for plans with Qualified Joint and Survivor Annuity)
Use this form to document the beneficiary for your retirement account. The Plan Sponsor should retain a current Designation of Beneficiary form for each participant in the plan.
